As a Nursing Home Administrator, your day is a constant juggle of budgets, staffing schedules, resident care plans, and family calls. While your facility may or may not have a dedicated Risk Manager, you know the truth: the buck stops with you.

You are the facility's de facto Chief Risk Officer, the person whose leadership sets the tone for safety, quality, and resilience. Proactive risk management isn't just about preventing lawsuits; it's the bedrock of excellent resident care, strong staff morale, and a sterling reputation.

This week, as we recognize the importance of health care risk management, take a moment to reflect on the pivotal role you play. Whether you're walking the halls or leading a meeting, you are constantly managing risk.

While not exhaustive, here is a quick-glance checklist for some risk management items organized by key risk domains. Use it to spark conversations with your team and reinforce a culture of safety.

The Administrator's Daily Risk Management Checklist

Think of this as a mental walk-through you can do anytime.

✔️ 1. Clinical & Resident Safety Risk

(The Core of Your Mission)

Fall Huddles: Are your teams consistently conducting post-fall huddles to identify the root cause? It's not just that a resident fell, but why. Are interventions appropriate and timely? Are care plans updated?

Medication Passes: When you observe a med pass, do you see staff focused and free from distractions? Are the 7 Rights being followed?

Care Plan Accuracy: Pick one resident chart today. Does the care plan accurately reflect the resident's current condition and needs? Outdated plans are a significant risk.

Infection Control: Are hand hygiene stations full and accessible? Observe staff practices—are they consistent?

✔️ 2. Operational & Environmental Risk
(The Physical Plant and Its People)

Walk the Halls (With "Risk Goggles"): Look for the small things. Are hallways clear of clutter and equipment? Are wet floor signs used properly? Any frayed electrical cords?

Emergency Preparedness: When was your last elopement or fire drill? Does your night shift know the protocol as well as the day shift?

Staffing & Ratios: Look beyond the numbers. Do your nurses seem rushed and overwhelmed? An exhausted, burnt-out team is your biggest operational risk. What's your contingency plan for call-outs?

Equipment Check: Are mechanical lifts, call lights, and bed alarms in good working order? A quick chat with your maintenance director can confirm PM schedules are on track.

✔️ 3. Regulatory & Reputational Risk
(Protecting Your License and Your Name)

The Grievance Process: Is your process for handling resident and family complaints clear, timely, and fair? A well-handled complaint can prevent it from escalating to a state surveyor or a negative online review.

Documentation "Spot Check": Remind your team: "If it wasn't documented, it wasn't done." Are daily notes thorough? Are incident reports detailed and objective?

Family Communication: Are you and your team communicating proactively? Bad news is always better delivered by you than discovered by a family member.

HIPAA & Privacy: Are nurses' stations secure? Are conversations about residents held in private spaces?

Putting It Into Action This Week

You don't need to add another meeting to your calendar. Instead:

· Theme your daily huddle: Focus on one category from this list each day this week.

· Empower your DON: Ask your Director of Nursing to lead a discussion on clinical risks.

· Walk with your Maintenance Head: Do an environmental risk walk-through together.

Being an administrator is a monumental task. By embedding risk management into your daily leadership, you aren't adding another "to-do"—you are simply doing your job with excellence.
