For three decades, I've witnessed the unwavering dedication of healthcare administrators, the unsung heroes navigating the complex landscape of long-term care. Yet, the whispers of exhaustion I once heard have now erupted into a resounding roar. Across the industry, a surge of administrators are bravely articulating their absolute burnout, a sentiment that deeply resonates with my own years of experience. The inquiries about alternative career paths, the willingness to accept significant pay cuts for a semblance of work-life balance – these are not isolated incidents, but rather stark indicators of a system straining its most crucial leaders.
The burdens administrators carry are immense. Beyond the paramount responsibility of ensuring quality resident care, a monumental task in itself, you serve as the emotional bedrock for your teams, tirelessly navigating staff retention crises and mediating family grievances that often extend far beyond reasonable expectations. The relentless pressure to inflate census, sometimes with potentially inappropriate admissions, further compounds the strain.
We are all too familiar with the challenges. Corporations leverage your expertise, often discarding you without a second thought after a Change of Ownership or a less-than-perfect survey outcome – outcomes that may have been influenced by factors entirely outside your control. Administrators frequently become the scapegoats, and the abrupt removal of leadership, devoid of judicious consideration, inflicts significant harm upon the facilities. This instability fractures teams, disrupts established systems and processes, and often precipitates a descent into chronic noncompliance.
Adding insult to injury, while corporate entities often remain shielded from direct scrutiny by state survey agencies, it is your license on the line. This inherent imbalance, this relentless pressure cooker, is precisely what fuels the urgent need for administrators to find their collective voice and demand better.

I vividly recall those initial, overwhelming moments: the handover of keys accompanied by deafening silence regarding training, the unspoken expectation of inherent knowledge. Your office transforms into a relentless revolving door of crises, each demanding immediate and unwavering attention. The meticulously crafted schedule, designed to address every facet of the day's demands, crumbles under the weight of corporate mandates for yet another report, an urgent ADR submission, a sudden legal notice, or the unsettling arrival of family or resident allegations.
And then, the ultimate disruptor: the regulatory body walking through your doors. Suddenly, the essential care and attention you diligently strive to provide to your residents often takes a backseat to the immediate and all-consuming demands of the survey process.
A severe disconnect, a chasm, exists between the expectations of operators and regulatory bodies and the practical realities on the ground. This complexity is further amplified by the often-conflicting guidance we receive. Consider the upcoming surveyor guidance on Enhanced Barrier Precautions (EBP), slated to take effect on April 28th, 2025. It mandates the use of EBP during transfers in shower rooms or therapy gyms but deems it unnecessary in the dining room. With three decades of experience, holding licenses as both an administrator and a nurse, I can unequivocally state: the inherent risk of a transfer, or lack thereof, remains constant regardless of its location.
Too often, the question echoes: "What else can I do?" The weight of responsibilities feels crushing, and the desire to abandon the industry becomes a persistent, tempting whisper. But the truth is, this industry desperately needs you. It needs individuals like you, those who possess a profound commitment to the well-being of others and dedicate their lives to their care.
This is not a moment for quiet resignation; it is a call to action. We, as administrators, can no longer afford to silence our voices for fear of termination or the inability to secure future employment. We must stand firm against corporate greed and mismanagement and fiercely advocate for ourselves, for our dedicated teams, and, most importantly, for the vulnerable residents under our care.
While state agencies representing operators can offer valuable input, I implore you to actively seek out and engage with organizations solely dedicated to administrators. Get involved. Your firsthand experience, your daily battles, are invaluable. You possess an intimate understanding of day-to-day operations that no one else can replicate.
When vendors go unpaid, when essential resources for success are withheld, it is time we, as administrators, begin to hold operators accountable. The increasing recognition by surveyors of administrator turnover as a critical indicator of systemic problems within a facility upon their arrival speaks volumes.
Operators must begin to treat administrators with the respect and support they deserve. They must provide the essential resources, ensure timely payment of bills, address critical plant operations issues promptly, and empower us to maintain regulatory compliance effectively.
